Once approved, you can choose OzPol as your Support at Home provider. We will work with you to develop a personalised care plan and coordinate the services you are approved to receive.

- Domestic assistance
- Cleaning and household tasks
- Home maintenance and repairs
- Gardening assistance
- Meals and meal preparation
Restorative Care Pathway
A setback doesn’t have to mean losing your independence. At OzPol Services, our Restorative Care Pathway is designed to help you regain confidence, improve your abilities and continue living safely and independently in your own home.
Whether you are recovering after a hospital stay, illness, injury or a decline in your mobility, our experienced team works alongside you to develop a personalised plan that focuses on your goals. We encourage and support you to do as much as you can for yourself, helping you rebuild strength, confidence and everyday skills at your own pace.
Our Restorative Care Pathway may include:
- Personalised support to achieve your individual goals
- Exercises and activities to improve strength, balance and mobility
- Allied health services, including physiotherapy and occupational therapy
- Assistance with daily living tasks while you regain independence
- Equipment and home modifications to improve safety
- Falls prevention strategies and confidence building
- Ongoing encouragement, education and support for you and your family
End-of-Life Pathway
When someone is approaching the final stage of life, having compassionate, trusted support can make all the difference. At OzPol Services, we are committed to providing gentle, respectful care that honours each person’s wishes, values and dignity, while offering reassurance to those who matter most.
Remaining in the comfort and familiarity of home can provide peace, security and precious time together. Our experienced nurses and dedicated care team work alongside your GP, palliative care providers and other health professionals to deliver coordinated care that is tailored to your individual needs.
End of Life Pathway can include:
- Personal care and comfort support
- Nursing and clinical care
- Medication assistance and monitoring
- Mobility and positioning support
- Meal preparation and hydration assistance
- Emotional support and companionship
- Respite for family members and carers
- Coordination with palliative care and other healthcare professionals
Assistive Technology and Home Modifications
The Assistive Technology and Home Modifications scheme provides separate funding for eligible Support at Home participants to access equipment, products and home modifications that support safety, mobility and independence.
Assistive Technology may include:
- Mobility aids and equipment
- Walking aids
- Wheelchairs
- Shower and bathroom equipment
- Adjustable or supportive equipment
- Products that assist with everyday activities
- Other approved assistive technology based on assessed needs
Home Modifications may include:
- Grab rails and handrails
- Ramps
- Bathroom modifications
- Changes to improve access and mobility
- Other approved modifications that make your home safer and easier to navigate
